Planning a Framework for Guideline Evaluation Before Publication of CDC’s HIV Self-Testing and HIV Screening Guidelines

Background: Successful dissemination and implementation of forthcoming Centers for Disease Control and Prevention (CDC)’s HIV self-testing and routine screening guidelines is crucial to improving early detection and reducing transmission of the virus. Providers are key to this success, but they may face challenges in adopting the new recommendations. Methodological advances over the years have significantly improved the way guidelines are developed, however, studies evaluating guideline uptake are lacking. A mixed-method approach applying a guideline evaluation framework was designed to gauge awareness, attitudes, and implementation barriers of the HIV testing guidelines among providers in different settings once the guidelines are published.
Methods: The data collection instruments (survey and interview instruments) were developed to assess the stages of guideline implementation and uptake. A thorough review of previous of guideline implementation experiences and existing literature was conducted to inform the development of these instruments. Additionally, a pilot study was carried out to ensure the survey was clear, concise, and easy to understand.
Results: This proactive evaluation initiative has yielded data collection instruments that consider key topics of interest. The survey collects general information on guideline attitudes, awareness, and adherence; as well as training needs and resources for providers. To collect more detailed information, the interview guides delves into the topic of guideline integration within clinical workflows, population health outcomes, and clinical decision support utilization. This instrument also measures facilitators and barriers to guideline adherence to include policy and regulatory factors. Ultimately, these instruments will serve as models for future evaluation efforts examining guideline implementation and translation on a national scale.
